When I had acne, I used to pop it. We all do it. But this is not the right way to treat. It may even worsen the pimples.

If you are also popping the pimples. Stop this. It may open the gateway for bacteria that become trapped in the layer of your skin.

As you know, acne is a very common problem among adolescents. It is really difficult to live with pimples which make you frustrate and lose your self-confidence.

In such a situation, the question often arises in your mind that how to remove pimples overnight.

There is no short cut. But here, I will provide standard clinical treatment options to remove acne. So, Let’s start the topic – How to remove pimple on the face.

Firstly, you need to find suitable anti-acne creams for your acne.

Although, there are numerous anti-acne creams available in the market. But the treatment depends upon the severity of acne.

The American Academy of Dermatology (AAD) has released a treatment algorithm for adolescents and young adult. These are first-line treatment options for managing different stages of your acne –

 

For mild acne (grade 1)

If you are having open and closed comedones with few red bumps (papules). You should start your treatment with benzoyl peroxide or topical retinoids (adapalene) or topical combination therapy

The topical combination may include –

  • Benzoyl peroxide + antibiotic (clindamycin); or
  • Benzoyl peroxide + retinoid (adapalene); or
  • Benzoyl peroxide + retinoid + clindamycin

 

According to a study published in Journal of clinical and aesthetic dermatology, 2019 addressed that combination therapy is considered the most effective treatment than a single topical agent.

Now, Let’s know the details of each topical agent.

1. Benzoyl peroxide

It is an OTC (over the counter) medicine which does not require a prescription. It is available in variable strength from 2.5 to 10%. It does not cause antibiotic resistance.

It is a powerful oxidising agent and bleaching agent which break down the keratin. It kills the bacteria of your skin as it also has bactericidal property (kill the bacteria). It removes the extra oil and dead skin from your face.

If you have sensitive skin you may suffer burning, itching and redness in the applied area.

 

2. Topical Retinoids

It is a very important agent to regulate your epithelial cell (superficial layer or topmost layer) growth.

Topical retinoids (like adapalene and tretinoin) binds to a specific retinoic acid nuclear receptor. It normalises your cell cycle of the skin and decreases follicular hyperkeratosis which prevents from microcomedone formation.

You should avoid this drug (either topical or oral) if you are pregnant or planning for pregnancy.

3. Clindamycin

It is a lincosamide antibiotic that inhibits bacterial protein synthesis by binding 50S ribosomal subunit.

Many studies suggest clindamycin induces antibiotic resistance which is an ongoing problem. So, you should prefer combination therapy instead of topical antibiotic monotherapy.

4. Alternative topical cream (Azelaic acid 20%)

It is not the first-line treatment but it can boost the effectiveness of other anti-pimples cream.

It is the best option if you are pregnant and while breastfeeding. It has antimicrobial properties and keeps your pores clear. It also manages the discolouration of your skin.

For moderate acne (grade 2 and grade 3)

If you are struggling from inflammatory acne lesions with comedones, several papules (red bumps) or pustules (yellow bumps) and few nodules.

You need to add oral antibiotic therapy with your acne creams. But you should consult with your dermatologist before taking oral antibiotics.

The treatment options maybe like this – 

1. Topical combination therapy –

  • Benzoyl peroxide + antibiotic (clindamycin); or
  • Benzoyl peroxide + retinoid (adapalene); or
  • Benzoyl peroxide + retinoid + clindamycin

2. Oral antibiotic + topical retinoid + benzoyl peroxide

3. Oral antibiotic + topical retinoid + benzoyl peroxide + topical antibiotic

 

Oral antibiotics –

As per studies, doxycycline and erythromycin are considered the first-line oral antibiotics for acne. These are the members of the tetracycline series of antibiotics.

These drugs inhibit protein synthesis by binding 50S ribosomal subunit. They help to kill P. Acne bacteria.

You should avoid doxycycline if you are pregnant or breastfeeding women. This drug may affect the growth of bones and tooth.

Doxycycline also avoids in children (less than 8 years).

For severe acne (grade 4)

If you are having highly inflammatory acne with large pustules, painful nodules and few cysts. Definitely, it is a serious concern and you must consult your dermatologist.

In this grade, the treatment maybe –

  • Oral antibiotic + topical combination therapy
  • Oral isotretinoin

Isotretinoin significantly inhibits the sebaceous gland activity. It decreases the size of the sebaceous gland and the production of sebum.

The exact mechanism of this drug is yet unknown but some studies suggest that it induces apoptosis to stop abnormal cells activity.

Isotretinoin is the most effective medicine in severe acne but it has serious side effects so you should take this medicine under medical supervision.

You should strictly avoid this medicine if you are pregnant or breastfeeding. This medicine is a powerful human teratogen which induces a high frequency of life-threatening birth defects. It comes in X category which is a high risk for pregnant women.

You should also avoid isotretinoin –

  • If you are having liver problems
  • If you are allergic problems to peanuts or soya
  • If you have been diagnosed with high cholesterol and triglycerides level
  • If you have been diagnosed with high vitamin A
  • If you are on treatment with tetracyclines

 

Top 10 ultimate tips to prevent your pimples –

Your medicines won’t work alone. There are some ultimate tips which enhance the effectiveness of your medicine.

1. First of all, you need to change your sedentary lifestyle.

2. You should avoid high glycemic index foods (chocolate, sweets, white bread etc.) which may exaggerate your acne problem. You may consider low glycemic index foods (vegetables, fruits, multigrain bread etc.).

 

 

 

 

 

3. The sweating during exercise may increase the build-up dirt in your skin pores. Despite this, you should not stop exercise. You can properly clean your face after exercise which prevents from clogging of pores.

 

4. You should avoid smoking as it releases free radicles and increases the sebum production that leads to clogging pores.

 

5. You should choose the best cleanser or face wash for your acne skin. Salicylic acid (0.5 to 5%) or glycolic acid is ideal for your blackheads/whiteheads. It removes the dead skin of your pimples and prevents the clogging of skin pores.

 

6. You should avoid cosmetics. You may use non-comedogenic moisturizer (oil-free) to prevent from dryness of your skin. You may also use a moisturizer which contains exfoliating agent (like lactic acid, glycolic acid, salicylic acid) or humectants (like glycerine, lactic acid, urea, hyaluronic acid and alpha-hydroxy acids).

 

7. You can use non-comedogenic sunscreens to prevent from exposure of sunlight.

8. Do not try to pop or squeeze your pimples. You should not touch your face unnecessary.

9. You should not forget to remove your makeup while going to bed.

10. You should not use much oil in your hair.

We have seen in this post (How to remove pimple on the face), there are various treatment options available for mild to severe pimples. It may undoubtedly cure. But you need to have patience as it takes 2-3 months to cure and may go up to 6 months.

Being a pharmacist, I recommend you that maintain your skin hygienically and do not build up oil or dirt in your skin pores.

You may use OTC medicines (like benzoyl peroxide cream) for your acne and do not try to take oral medicines yourself as it may have serious side effects. You must consult to a dermatologist if there is high severity of acne problems.
